The one-page document is accompanied by a notarized affidavit from Alan Barcoff, a good will ambassador for Bad Boy Records from 1997 to 2007; a printout of an image of Barcoff with Bad Boy Records founder Sean "Diddy" Combs; and photocopies of invitations to company events.
