The first, a document headed "Affidavit of Birth" for Lucille LeSueur (Joan Crawford), is dated 26 September 1942. The second, a document headed "Affidavit of Birth to be Submitted with Application for American Passport," is dated 17 March 1939. Both documents are signed "Anna LeSueur" and are clipped to a carbon copy of a letter from Albert J. Lewis, Joan Crawford's business manager, to the County Clerk's Office in San Antonio, Texas, relaying their desire to obtain a copy of Crawford's birth certificate.
