A pair of hospital documents for Kurt Cobain dated April 15, 1991, from St. Peter Hospital in Olympia, Washington. The first is a Diagnostic Imaging Procedure Form that lists Epigastric pain as the reason for the visit and Cobain's escort as "David Grohl, friend." The second is a carbon copy of the Patient Report and Discharge Instructions with imprinted signature of both the doctor and Cobain. The doctor wrote a conclusion of "Acid irritation - no tumor," with instructions to call "when your head clears."
